Job Title: Social Worker, Barnet and Chase Farm Hospital Team
Location: Barnet General Hospital
Salary: H8 – M1 £27,741 - £39,880pa plus 6,000 relocation package, £1000 welcome payment & lease car* terms apply and £4,000 Market Forces payment 
Hours: 37 per week

Barnet and Chase Farm Team need enthusiastic, dynamic and resilient Social Workers.  You will need to be SWE registered and have knowledge and experience of relevant legislation. We are looking for a experienced qualified SW with at least 18 months experience. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. 

At Barnet and Chase Farm Team we promote personalisation focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by enable people to direct their own support. 

You will gain extensive experience in the field of Adult Social Care, Safeguarding practice and Multi-Disciplinary working.

There are a large number of complex and vulnerable patients within the hospital who require our support to get home as quickly as possible.  Achieving this for each patient requires talented staff with a blend of skills. Each case has to be managed carefully using strong professional judgement and in a proactive and tenacious way in order to take responsibility for efficiently enabling patients discharge.

Social care is vital to the successful and safe discharge of people back to their own home. Supporting people with the right care and information at the right time can help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.

The post may require you to visit service users in the community to undertake reviews, so a driving licence would be essential as is the ability to respond to situations quickly and flexibly.

To improve outcomes for service users, the Integrated Hospital Discharge Service supports a 7 day Hospital Discharge service.  The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends. There will be opportunities for you to discuss this at interview if your application is successful.
