Reassessment Team Adult Social Care:

Adult Social Care is recruiting a Lead Practitioner to join the established Reassessment team. Yr You will support the team manager in the development and management of the team in order to deliver high quality reassessments and support plans that address the needs of vulnerable people through commissioning individual packages of care that lead to improved outcomes whilst managing the reputation of the Service and the Council. Supporting practitioners in managing complex cases and providing formal supervision and deputising for the team manager as required.
This is an excellent opportunity for a qualified and experienced social worker with proven leadership ability.
You will have a thorough knowledge of adult social care legislation and statutory guidance will ensure that risks faced by vulnerable people are appropriately managed and team members from other professional backgrounds are supported in the delivery of services.

Due to the nature of work the above post is exempt from the provisions of the Rehabilitation of Offenders Act 1974 and / or relevant care / safeguarding legislation and will only be appointed if a satisfactory Enhanced Disclosure is obtained from the Criminal Records Bureau.
