 We care about the wellbeing of Berkshire residents. And our dedicated Adult Safeguarding & Hospital Discharge/Avoidance Officer service provision helps us to protect some of our most vulnerable residents. We work with health colleagues, families and the police to assess, plan, implement and evaluate treatment to help people live as independently – and fully – as possible. Our Adult Safeguarding & Hospital Discharge/Avoidance officers in the Emergency Duty Service specialise in assessing and managing the risk involved in protecting vulnerable adults, distinguishing levels of risk for different situations to prevent harm and abuse as early as possible. It’s a challenging role. But you’ll have excellent support from your manager, and expert guidance on hand whenever you need it. You’ll also have the time to go above and beyond, along with the chance to express your ideas about how we can make our service even better.

You will be supported to carry out the following duties, responsibilities and training…

You will be supported to work as a part of a varied and dedicated team in an environment that will provide you the opportunity to develop new skills as an adult safeguarding practitioner.

  • To work innovatively and proactively for an excellent out of hours social care service.
  • Explain clearly, and with sound rationale, highly complex safeguarding information to a wide range of professionals and to participate in the multi-agency risk analysis to inform case management advising where necessary and challenging decisions where appropriate
  • To provide information to service users through liaison with internal and external agencies to enable service users to access relevant services, and to encourage their participation
  • Ensure that the requirements of the Care Act 2014 are met and that vulnerable adult’s rights under the Act are upheld ensuring all requirements of the Care Act and associated legislation including case law are understood.
  • To maintain records and write reports in accordance with Departmental Policy and Practice and ensure clear, concise, and accurate recording of work undertaken, and good electronic and paper file management is maintained

What we’re looking for…

We are looking for an experienced practitioner who has excellent risk assessment and analytical skills, who is good at effective multi-agency working, good oral and written communication skills, a team player, with the ability to prioritise and organise work effectively. We are looking for a practitioner with good ICT literacy, who is able to deal with unrestricted demand & make decisions under pressure and who is willingness to challenge professionals and public. The service requires someone with passion for good social work and who is able to support their colleagues
