The SMART Service (Supportive Multi-Agency Response Team) is looking to recruit an experienced Social Worker for this brand new and innovative team

The Post

This is a new and unique service in the London Borough of Brent and has been created to provide targeted support for individuals with intersecting complex needs, particularly those with vulnerability and experience of exclusion that impacts on their mental health and wellbeing. These individuals would not normally be eligible for support from adult social care, as they do not meet the criteria set out in the Care Act. They may be under primary care for mental health, and are less likely to be managed long term under secondary mental health, potentially due to their reduced ability to engage.

The SMART Service is a creative response to work with people within the Borough who often have a cycle of homelessness, mental health admissions, drug and alcohol dependencies and contact with Criminal justice and probation services. Their multiple complex needs requires a more holistic response in order for them to be able to move forward and until now there has not been a targeted provision for this group of people.

The Person

We are looking for an experienced social worker who has worked effectively to support people with mental health problems and complex social problems in the community. Utilising your social work strengths in communication and engagement you will support service users who traditionally have found it difficult to maintain a relationship with professionals. Working as part of a mini team who will wrap around the service user, you will support the service user to identify their needs, stabilise and move forward with their life.
We will be utilising a strengths based approach in our work with complex service users with multiple needs. We are looking for a social worker who is creatively able to comprehensively assess people’s vulnerabilities and risks and identify ways in which service users can be supported.
You will assess people who are marginalised and difficult to engage, they may be Care Act eligible and at risk of significant harm. You will utilise your social work skills to identify and mobilise services, commission packages of care and safeguard our service users.

You will need to be able to work creatively with an assertive outreach approach.
