Job Title: Advanced Practitioner – Herts Valley Hospital Integrated Discharge Service
Location: Watford General Hospital
Salary Range: M1 - M2 £35,745 - £43,857pa + £4000 Market Forces payment + £1,000 welcome payment.
Hours: 37 hours per week
As an Advanced Practitioner you will already have the knowledge and skills of a social worker and be able to take on a consultative role for less experienced staff. You will use advanced expertise and knowledge to take on management of the most complex cases and to contribute to team development. The Advanced Practitioner leads on peer development and learning in conjunction with development programmes and workforce strategy and provides coaching, mentoring and supervision as appropriate. You will need to be a registered Social Worker and have knowledge and experience of relevant legislation.
There are a large number of complex and vulnerable patients within the hospital who require our support to get home as quickly as possible. Achieving this for each patient requires talented staff with a blend of skills. Each case has to be managed carefully using strong professional judgement and in a proactive and tenacious way in order to take responsibility for efficiently enabling patients discharge. You will undertake the more complex cases within the team. You’ll also have the ability to supervise others to enable us to continue to achieve excellence in the delivery of our frontline services.
The post may require you to visit service users in the community to undertake meetings, so a driving licence would be advantageous as is the ability to respond to situations quickly and flexibly.

To improve outcomes for service users, the Integrated Hospital Discharge Service supports a 7-day Hospital Discharge service. The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.
