Job Title: Advanced Practitioner in the Integrated Hub

We are looking for a qualified social worker, ideally with experience of working in an integrated setting.  This is an exciting new role working closely with Health colleagues to support prevention of admissions to hospital and enable people to remain in their own homes where possible.  Your primary role will be to triage social care referrals, provide information and advice and establish the appropriate pathway for the individual.  You will work across the East and North Hertfordshire localities.

This is an evolving role with the potential to expand the social care service within the Integrated Hub to provide a streamlined referral pathway for individuals.  You will have the opportunity to influence and be of the development of the social care role within the Hub.

The Hub is currently based at Robertson House, Stevenage however we you will have the opportunity to work flexibly with the benefit of a laptop and smart phone so that you have the option of working at home or at one of our touchdowns across the area.  

We are looking for a dynamic social worker who have a commitment to supporting the local community and building links and networks with our partners in health.  You will need to be Social Work England registered and have knowledge and experience of relevant legislation.
