About the role
Brighton and Hove is a vibrant and exciting place to live and work and an opportunity has arisen to join the Hospital Responsive Service social work team.
Both the acute and community arms of our wider team have seen their roles significantly evolve in recent years as we have been at the forefront of changes with the hospital’s Discharge to Assess model. This has meant our team work with people in the acute setting as well as those in community settings after a period of time in hospital. This can be either in health beds whilst the person is benefiting from rehab or in the person’s home. We also work with those who need support to prevent a hospital admission including playing a role at the front door of the hospital by working with our health colleagues in the Emergency Department.
We are a friendly and supportive team working in a busy environment; although working under pressure we maintain high professional standards of practice and are committed to ongoing professional development. The successful candidate will be expected to work across all settings.
For an informal discussion, please contact Anna Curtis, Operations Manager or Jane Higgins, Operations Manager on 01273 696955 ext 4001.
