Salary Range: £35,745- £43,857 per annum - dependent on experience - plus £4k market forces payment
Hours: 37 per week
Contract: Permanent

0-25 Together is Hertfordshire County Council’s multi-disciplinary Social Work and Adult Nursing Service that supports children, young people and families with a range of complex needs such as disabilities, or life-limiting or lifelong support needs from birth to age 25.

About the role
Joining us as an Advanced Practitioner, you’ll lead and develop practice to enhance personalised approaches to service delivery through the local community with our young adults.  In particular, you will work to build community capacity and involvement around service users, and ensure they have the level of support they need to lead safe, happy and healthier lives in the way they choose.

You’ll provide expert information, advice and guidance in complex situations, as well as assessing, planning, commissioning and brokering services to meet the needs of your service users and deliver good outcomes.

About the person
A qualified and registered Social Worker or Qualified Occupational Therapist, you’ll have experience of working with vulnerable adults within a health and social care setting.  You’ll be used to multi-agency work and to finding innovative ways to meet the needs of service users.  Knowledge of safeguarding and adult social care legislation is essential. You should also be capable of supervising students, and coaching and mentoring others.
