Borough Discharge Support Officer x 2

Woking Borough Council is recruiting two full time Borough Discharge Support Officers to work with our health and social care partners to assist timely discharge from acute hospitals with a wraparound District and Borough Support offer.   

Working as part of a wider Integrated Discharge Team, the post holders will support patients through the discharge pathway and work with residents on discharge support plans, ensuring that discharge is an efficient and effective process and that patients have the support needed to continue to live independently.  

Applications are welcomed from individuals with expertise in dealing with residents in times of crisis, hospital discharge and partnership working. You will be managed in our Social Prescribing team delivering a personalised care model mostly based at Ashford and St Peters Hospital in the Integrated Discharge Hub and Clinical Assessment Unit and other acute sites in the area supporting Woking, Runnymede, Spelthorne and Elmbridge residents. 

The successful candidates will have good time management skills, work well under pressure, be a team player, have a desire to help residents, make a difference and be confident to make decisions and make partners act. The role requires the ability to be able work with your own initiative on a rota basis that will include some weekend and evening work as well as across varied teams in multiple organisations, including partner Borough Councils and hospital based teams.
