Team Manager

We are looking for an additional Team Manager to join our Herts Valleys Post Hospital Review Team (PHRT). The role will be based in Apsley, Hemel Hempstead.

With a professional qualification/registration in Social Work, Nursing or Occupational Therapy you will manage a team of skilled professionals including Social Workers, Community Care Officers, Enablement Community Care Officers, OT, Care Choice Facilitators and Home Finders. You will ensure the teams roles and responsibilities are undertaken effectively and efficiently on a day-to-day basis to support Care Act assessments and progress ongoing care plans for those who are discharged from hospital on transitional (enablement) pathways. We oversee the ‘Discharge to Assess’ Pathways and have a strong ‘Home First’ and enablement ethos.

During the past few years we have seen the positive outcomes of supporting people on transitional pathways from hospital to home by completing all follow up work and assessments outside of the hospital setting. Due to success this has resulted in the PHRT requiring additional management support.  

On a day to day basis, you will plan, allocate and review workloads, manage priorities and implement and improve systems and processes to achieve performance targets. You will have extensive knowledge and experience in the field of adult social care, with an emphasis on personalised practice and related legislation.  You will have the ability to manage competing service priorities and a sound grasp of current legislation, the personalisation agenda and meeting the business objectives of the department.  Demonstrating management potential, you will have excellent all-round communication and negotiation skills in leading an experienced staff team and in working with families / carers, Statutory, voluntary and independent sector partners.

To improve outcomes for service users, the Post Hospital Review Team supports a 7 day Hospital Discharge service, working 8-8 weekdays and 9-5 weekends. The post holder will play an active part in the service offer to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering weekends.
