Following the redesign of the Social Work Teams within Focus and internal progression of one of our team members, we are looking for Level 1 Practitioner to join the Triage Team.
The function of the triage team is to prioritise and manage all referrals coming into Adult Social Work, and through the multidisciplinary Single Point of Access (SPA), essentially via the telephone. You will provide professional advice and guidance to colleagues and facilitate proportionate holistic assessments both face to face and over the telephone, allocate work to social work colleagues, liaise with other agencies and professionals to ensure most appropriate outcome for the individual/ carer is paramount.
The SPA and Triage team are constantly striving to enhance their knowledge and practice. Our work is strength based and solution focussed, which promotes independence and resilience within individuals and carers that contact our service.
Part of your role will be to continue developing these approaches, building your own knowledge and assisting the Level 2 Practitioner, in the development of the wider team.
You will need to be able to build positive relationships at a number of levels and bring enthusiasm, creativity and passion to the service. If you are motivated, reliable and able to work autonomously and find creative solutions for the people you work with, then you are the social worker who we want to work in our team.
You will need to be a registered Social Worker with Social England, and have at least 12 months post qualifying experience as well as completed your Assessed & Supported Year of Employment (ASYE).
**From 1 September 2022, the full time working week at Focus will be 34 hours per week **
