The 0-25 Together Service is a fantastic example of what happens when likeminded people come together to deliver specialist services for disabled children, young people, young adults and their families. We want our service users to enjoy as much independence as possible, while reaching their full potential. We want them to be able to take part in their local community and find stability and fulfilment as they move into adulthood. Working in partnership with everyone who comes to us for support, we pride ourselves on being there for them as they settle into successful adult lives.

We are now recruiting for Social Workers to join our two teams. One of these teams provides a high-quality level of support to adults who have complex needs, while the other team works with disabled children and young people. Regardless of which team you join, you will assess the needs and circumstances of an interesting range of cases, endeavouring to help people establish independence (either now or in the future) while balancing this with safeguarding considerations. Providing a great level of support to service users’ support networks, you will advise them on resources within the community that may be helpful, and you will collaborate with both statutory and voluntary agencies to deliver great life outcomes. Every day will present you with new challenges, as you could handle anything from making recommendations on home adaptations through to enrolling service users in enabling programmes. From your initial assessments through to overseeing the planning and delivery of services – and then monitoring the success of these measures – you will play a vital role in keeping people happy and safe.

If you are ready to take on one of these unique roles, you will need to be a professionally qualified and HCPC registered Social Worker, with post-qualification experience. With plenty of resilience and a patient approach, you will be capable of handling difficult situations in a calm manner, and you will have the confidence to provide advice and guidance to colleagues and external partners. With good IT, verbal and written communication skills and the capacity to identify different ways of communicating depending on who you are talking to, you will have an up-to-date knowledge of the relevant legislation. If you identify a situation in which a person’s health or capacity is deteriorating (or is likely to deteriorate) you will devise creative interventions, so a great deal of tact will be required to excel in this role. Crucially, you will be keen to maximise independence wherever possible.
