Job Title: Adult Social Workers
Location: Countywide 
Starting Salary: £33,820 with the opportunity to progress to a maximum of £37,261 per annum plus relocation package, £1,000 welcome payment* terms apply & lease car* terms apply plus £1,591 market forces for ASYE Social Workers or £3,441 market forces for Social Workers who have completed their ASYE year.
Hours: Part time and 37 hours per week
Contract: Permanent and fixed term contracts available

  • Integrated Discharge - Princess Alexandra Hospital (based in Harlow Essex)

We support Hertfordshire residents to return home safely.  Working with their strengths, community resources and if needed a support package. We work with the multi-disciplinary team to understand the needs of our service users that we work with.  If you are interested in working in a hospital environment and are prepared to work quickly and learn a lot this job may suit you.

  • Integrated Discharge - Post Hospital Review Team /  Lister / Community Hospital Team

The successful applicants will able to evidence sound knowledge of safeguarding practice, working with people with dementia, the personalisation agenda and Individual Budgets. Able to work in a fast paced team You will keep the user and any future potential users at the centre of all activity and address user needs within the legislative framework. You will be responsible for assessing and arranging care in a hospital and community setting. We are a busy team covering a large and diverse area and would welcome applications from candidates who can bring energy, creative care planning and are citizen focus.

We promote personalisation focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by enable people to direct their own support. You will gain extensive experience in the field of Adult Social Care, Safeguarding practice and Multi-Disciplinary working. There are a large number of complex and vulnerable patients within the hospital who require our support to get home as quickly as possible.  Achieving this for each patient requires talented staff with a blend of skills. Each case has to be managed carefully using strong professional judgement and in a proactive and tenacious way in order to take responsibility for efficiently enabling patients discharge. Social care is vital to the successful and safe discharge of people back to their own home. Supporting people with the right care and information at the right time can help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.

  • Watford Hospital, Langley House, Herts Valleys Post Hospital Review Team, Out Of County Hospital, Community hospital  

The Herts Valleys Integrated Discharge Service comprises of teams both in acute hospital settings and in the community, working to a ‘Homefirst’ agenda to support Hertfordshire residents being discharged from hospital. Your role would include undertaking care assessments that apply creative problem solving and strengths-based practice. Our focus is to support recovery and maximise independence following hospital admission.
