Communities and Prevention Coordinator

This role has a full time salary of £31,262 per annum, it will be pro rated depending on how many hours are worked a week.

We have a variety of vacancies in multiple locations;

26 hours per week - £22,578 per annum - Runnymede Civic Centre, Station Rd, Addlestone KT15 2AH - and - Quadrant Court, 35 Guildford Rd, Woking GU22 7QQ

18 hours per week - £15,631 per annum - Spelthorne, Knowle Grn, Staines TW18 1XB

14 hours per week - £12,157 per annum - Elmbridge, Civic Centre, High St, Esher KT10 9SD

15 hours per week - £13,025 per annum - Mole Valley, MVDC, Pippbrook, Dorking, Surrey, RH4 1SJ

As a team our ambition is to reduce health inequalities, promote independence and achieve better outcomes for people. We want to embed a model of social care that grows local resources and encourages community behaviours which benefit the resident and enables the council to deliver services within the resources available.

This role will support an environment where residents with social care needs can be diverted towards informal, free or low-cost community-based support that prevents, reduces or delays care and support needs from developing, as required by the Care Act 2014.

In this role you will be responsible for maintaining databases of information regarding local voluntary sector support, community networks and possible solutions. You'll set up and work on local and county-wide prevention projects with local partners to develop a range of community assets to improve wellbeing and you will receive individual referrals from Social Care Teams supporting practitioners to identify community resources for service users.

Additionally, you will work with voluntary and community groups to build inclusive and self-sustaining models of support, which reduce stigma, promote independence, tackle health inequality, and achieve better outcomes for our residents.

In order to be considered for shortlisting for this role, you will evidence that;

  • You have knowledge and experience of working with adults with social care needs in a health or social care setting.
  • You are willing to travel across Surrey to community locations that may not be easily accessible by public transport.
  • Awareness of the principles of confidentiality and the importance of complying with confidentiality policies within the service.
  • Basic understanding of common health conditions, associated impairments, mental health, and the disabling barriers individuals may experience
  • Experience of working with communities and voluntary organisations
