Multi Disciplinary Working in Lewisham Adult Social Care

We are looking for skilled social workers and social work leaders who have the experience of working across the Health and Social Care Services in adult social care. The Roles

We have a number of permanent social work roles that need to be recruited into enabling the opportunity to work within our Discharge 2 Assess (D2A), Urgent Care Response (UCR), Complex Discharge and Continuing Heath Care (CHC) Teams.

D2A

The Discharge to Assess (D2A) pathway is an integrated multi-Disciplinary service which provides support and rehabilitation to adult Lewisham residents to return them to an optimum level of function after an admission to hospital.

The team is made up of staff from health and social care (social workers, therapists, and nurses), working together to provide a holistic assessment and support plan to ensure outcomes are achieved and independence is maintained wherever possible. In this role as a Social Worker, you will be ensuring that citizens with care and support needs receive community care and support in accordance with the Care Act 2014 always ensuring and strength-based approach and person-centred support plans. Additionally, you will be identifying and carrying out Mental Capacity Assessments and chair best interest meetings as well as protect adults at risk of abuse in accordance with Lewisham Council Safeguarding Policy, the Care Act 2014 and making safeguarding personal.

This pathway is a 7-day service. There is a duty rota in place which will allow you to both working from home and being based in our main office (Nursery Building – University Hospital Lewisham). You will be working 35 hours per week in a very diverse and inclusive environment which is reflected in our teams and where we create greater opportunities for professional growth.

CHC

The Continuing Health Care pathway is part of the service which deals with individuals who may require long-term 24-hour placements. This is a complex pathway which involves completing Care Act Assessments mental capacity assessments, best interest decisions, and completion of Decision Support Tools (DSTs). with a view of presenting cases to CHC panel for funding ratification.

Hospital Based Complex SW team

The Complex Discharge team is part of a wider Flow Centre/TOC, with a collective “Home First” ethos and strength-based approach to discharges and managing complex cases. The Teams undertakes the work to facilitate safe and timely discharges for Lewisham Residents in University Hospital Lewisham and Out of Borough Hospitals. The Team looks to sustainably support people being discharged home or other settings. This will involve being ward liaison social worker within the trust, daily attendance at white board meetings as well as attendance at weekly multi-disciplinary meetings (MDTs). This also, involves the early identification of issues that may delay discharges such input around blitz cleans, complex family dynamics, homelessness, NRTPF, Mental Capacity Assessments and where required best interest decision meetings.

You will be expected to work in a Person Centred and enabling way to prevent any delays in transfer of care from the acute setting and ensure a positive Patient experience.

UCR

The Highly regarded Lewisham team supporting the primary care teams and London Ambulance Service is a multi-disciplinary team supporting to assist Lewisham residents in avoiding hospital admission and remaining independent longer It comprises of advanced health professionals, therapists and Social Workers

  • Our Hospital Team, is working to develop strong, effective partnerships with our health colleagues to proactively meet the outcomes & improve wellbeing for our service users.

     
  • All of this is underpinned by a solid safeguarding practice working in collaboration to develop a preventative approach to safeguarding in Lewisham backed by proactive Lewisham Adults safeguarding Board

Location 

The job is primarily based in University Hospital Lewisham, but we operate a hybrid approach where you will be required to work both in the office and from home on a regular basis.

Special Conditions

  • Current registration with Social Work England

     
  • 2 years post qualifying UK experience

     
  • Qualified Social Worker BA /DipSW/CQSW or equivalent and has completed ASYE programme

     
  • Weekend working may be required on a limited basis required for this job

     
  • The job is subject to an enhanced DBS check

Salary

£40, 503 - £42, 510 per annum (SW)

N.B: All appointments will be offered at the minimum spinal column point (SCP) for the grade of the role. The only exception to this will be when matching current salary (up to the maximum of the scale), which is subject to evidential proof.
