Job Duties:
This is an exciting opportunity to join Hertfordshire’s Community Learning Disability Nursing service in Hertfordshire County Council. As a Band 6 Nurse you will work with other nursing colleagues within the Adult Disability Team to reduce health inequalities for people living in Watford & Three Rivers.

As a Community Nurse you will work alongside paid and family carers to support people with learning disabilities to access mainstream health services by supporting clinicians to make reasonable adjustments. Your skills in adapting and creating the right approach for the individual will help bridge the gaps and reduce the risk of avoidable health conditions.

We require people who can be creative and flexible to different approaches for different situations as no day in community learning disability nursing is the same. As the post is in the community you will need to be able to demonstrate the ability to travel to community visits and across to the 2 main HCC sites in Hemel Hempstead and Stevenage in a time efficient way.

As a Band 6 Nurse you will be responsible for triaging and managing referrals to the service along with supervising and supporting junior colleagues. You will also be required to utilise your skills as a Practice Supervisor and work towards becoming a Practice Assessor for Student Nurses on placement with the service. We welcome applications from nurses who are not a registered learning disability nurse if you can demonstrate experience and an understanding of the issues for this population.
