We are seeking an enthusiastic Social Worker to join a well-established Weston Adult Social Care Team based in Town Hall, Weston-super-Mare, North Somerset. The team covers most of Weston and part of Worle area. Duties include care assessment and arranging services for adults, who are experiencing problems relating to ageing, physical/sensory impairment and other serious health and social care problems.
Applicants need to be qualified Social Workers, registered with HCPC ideally with experience in Care Management, Safeguarding Adults and Mental Capacity Act processes. The team comprises of Social Workers, Occupational Therapists and Adult Social Care Workers. We work closely with our Health provider - North Somerset Community Partnership, Mental Health provider- AWP, Voluntary agencies and continue to look at developing and delivering a partnership approach to service provision.
This is an exciting and challenging time for Social Care as we head towards a structural change and to digital age involving exploring different ways of working to increase efficiencies. We offer Assessed and Supported Year in Employment (ASYE) to newly qualified Social Workers.
The successful candidate will join in Weston Adult Social Care Team based in Town Hall, Weston-super- Mare but may be required to work in other locations in North Somerset if there is a greater service need.
