For over 30 years, we have supported disabled and disadvantaged people to make positive changes to their lives through training and employment, and our ambitious plans need the right people to help us transform many more lives, in many more ways. 

Working withPrimary Care, Community Health Teams, and the wider community, you will deliver employment support to clients in parallel to the health treatment being delivered by NHS clinicians through the Thrive Individual Placement and Support Project. This project is an exciting new experimental trial, testing a new employment support service for people with a health condition. It aims to find out how good this new service is at helping people to find work. 

Working intensively with a maximum caseload of 25-30 clients, you will assist them in obtaining and sustaining employment that is consistent with their vocational goals, which will include referring them into the trial. You will complete extensive outreach activities to secure referrals to the trial and support them throughout. 

With significant experience of people development on a one-to-one basis, you will be client-focused with a strong commitment to client care. The ability to create and deliver action plans for clients is essential, along with demonstrable experience of supporting clients with health conditions and disabilities. First class communication skills are essential, as you will be dealing with clients and employers alike on a daily basis, as is a strong team ethic and positive and flexible approach.
