Team Leader Reablement & Rehab Team
Salary:£40,200 - £47,057 per annum
Hours Full Time:36hrs
The London Borough of Bromley Education, Care and Health Services are looking for a Team Leader to manage and support the Reablement and Rehabilitation Care Management team and also work closely with Direct Services and our Health partners.
You will be leading and supporting staff within the Function and working closely with Bromley Health Care colleagues and other professionals. You will also be managing two care managers who are working within the Neuro and Stroke pathway.
We are looking for somebody suitably qualified with experience at Team Leader Level or a Senior OT or Senior Care Manager who feels ready to rise to the challenge. We are also looking for someone who can work in a busyenvironment and who has the skills, knowledge and expertise to manage staff and the service and make sound decisions. You will work as an integrated member of the Function and ensure that the service delivers high quality, timely and proportionate assessments and interventions for adults in need;also analyzing the Reablement data to capture the throughput of cases including outcomes.
You will possess excellent communication, analytical and written skills, be a creative thinker, problem solver, as well as being effective at prioritizing, meeting performance targets and managing systems. You will have very good interpersonal skills
As part of a supportive management team, you will work to the requirements of the Care Act, along with best practice, regular supervision, training, career development and the Council’s Discuss model.
You must be a qualified Social worker, OT or Nurse with HCPC or NMC registration. Asatisfactory Disclosure Barring
