Wellbeing Adviser – Social Prescribing

Salary:                        £24,166 - £28,388 plus £2,000 per annum car allowance

Hours:                        37 hours per week, Monday to Friday

Contract:                    Fixed Term – 12 months

Surrey Heath Borough Council is looking for someone to lead on the delivery and development of the Social Prescribing service.

This will involve signposting adults who have been identified as mild to moderately fail and socially isolated or those that can benefit from the Social Prescribing service to social, recreational and non-medical support within the community which may help them to improve their health and wellbeing. You will be required to meet with residents for one-to-one appointments to discuss local sources of support that may benefit them.

The role involves working with the Social Prescribing partnership in Surrey Heath to collaboratively discuss, allocate and feedback on all referrals generated. You will be required to attend regular meetings to do this and to share knowledge and experience for the benefit of all partners.

You will be responsible for the day to day coordination and delivery of the service, from receiving referrals, carrying out appointments, following up assessments and completing all administration and monitoring.

To be successful in this role you will need to have a support or advice background and have an understanding of the needs and problems older, vulnerable and disabled adults may have. Knowledge of the range of services available locally within the borough to support this would be desirable but is not essential.

You will be organised, able to work independently and manage your time well to ensure you are able to meet changing workloads. You will need to be confident in using computers and in completing notes and spreadsheets accurately in order to manage the coordination of the service.

You will be confident in communicating with individuals on a one to one basis and be able to adapt your communication style to individuals who may have a variety of needs. Your empathetic, caring and personable approach and calm manner will be key to this role.

You will need to have a full driving licence with access to a vehicle on a daily basis. You will be required to undertake a DBS check.
