Empathetic and able to communicate with customers, carers and professional to ensure the best possible outcomes for those you work with.

What we need from you… With experience of carrying out assessments and reviews of care provision, you will be able to respond to changing needs and ensure the appropriate arrangements and adjustments are made in a timely manner. Using your knowledge of the Care Act 2014, and excellent organisation skills, you will ensure all reviews are carried out in a person centred, outcome focused way, and within set deadline

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
