Permanent Full Time, 37 hours per week

Scale 6 – PO1 (Career grade)

Salary £24,799 - £34,788 pa plus market supplement of £1,000 - £3,000 pro-rata for SW qualified

What we need from you… You are experienced in an adult social work/adult health environment, have a person centered, preventative approach and you are passionate about making a difference. You will have an understanding of the personalisation of services agenda, and the implementation of self-directed support services and be able to deliver the statutory functions of The Care Act, manage a caseload and be part of a motivated and highly experienced multi-disciplinary team.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
