We are seeking an experienced Social Worker to join our supportive and forward-thinking Exeter Central and East Community Health and Social Care Team, based at Whipton Hospital, Exeter.

About the Role

In this full-time position, you will play a key role in delivering high-quality, person-centred social work practice across our local community. You will work with adults with a range of complex needs, empowering individuals to live independently and safely while ensuring statutory duties and best practice standards are met.

As a Social Worker within our integrated Community Health and Social Care Team, you will undertake strengths-based assessments, care and support planning, Mental Capacity Act assessments, safeguarding enquiries and reviews. You will work closely with individuals, families, carers and partner agencies to identify solutions that build on people's strengths and help them achieve the outcomes that matter most to them.

The role offers a varied and rewarding caseload, including supporting people with physical disabilities, long-term health conditions, frailty, mental health needs and complex social circumstances. You will be responsible for managing risk, promoting choice and control, and ensuring that individuals receive timely and proportionate support.

You will be joining a collaborative multidisciplinary team that includes Social Workers, Occupational Therapists, Community Healthcare Professionals and other key partners. Together, we strive to deliver coordinated, joined-up services that place the individual at the centre of everything we do.

Social Worker Career Pathway

We have implemented the first phase of a new Social Worker Career Pathway within our Adults Services, with our vacancies available at the Social Worker (Grade F) or Experienced Social Worker (Grade G) levels. Our Social Worker and Experienced Social Worker roles align with the national Professional Capability Framework (PCF) for Social Workers, which provides a robust structure for all local authorities to use as a way of standardising professional capabilities and provides a clear career pathway.

This is a fantastic opportunity to provide you with an achievable, clear pathway for your career; to enable you as a practitioner to reflect and identify your own practice and development on the PCF pathway.

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).

With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
