The Bracknell Adult Community Mental Health Team is a dedicated team providing vital support to adults experiencing significant mental illness, as well as their carers. We offer a range of interventions, including medication management and psychological therapies, all while coordinating comprehensive, person-centred care within the community. The team also fulfils statutory responsibilities under the Mental Health Act 1983, the Mental Capacity Act 2005, and the Care Act 2014. We are committed to safeguarding vulnerable adults, actively identifying and responding to risks to ensure their safety and wellbeing.
<h3>Key responsibilities…</h3>

<li>To collaborate with the CMHT Senior Management team and Clinical Leadership to provide guidance on the integration of social work and social care values, practices and statutory responsibilities in the delivery of integrated health and social care services for adults with mental health needs.</li>
<li>To supervise the Social Work Lead, AMHP Lead and social care staff, whilst establishing a workload management system to ensure caseloads and workloads align with seniority, experience and are regularly monitored based on local needs analysis.</li>
<li>To lead and support delivery of statutory functions (Care Act, Mental Health Act, Mental Capacity Act, and s117 MHA) using strengths-based and recovery models.</li>
<li>To oversee social care budget and authorise payments in line with the Scheme of Delegation.</li>
<li>Take overall responsibility for team management including recruitment, supervision, appraisals and development of social care staff, apprentices, students and trainees, ensuring performance aligns with service priorities and individual growth needs.</li>

About the role
This is an exciting new role within a brand new contract to support the referral process from the Housing Solutions teams! The service focuses on providing floating and wraparound support to single applicants who are at risk of homelessness and are able to live independently, by providing a housing support plan towards resolving housing needs and support them into long-term, stable housing.
In this role, you will conduct proactive outreach with our participants to develop personalised support plans for housing for appropriate long-term housing options, including accessing private rental listings and understanding application processes for tenancy. You will help our participants navigate the resources available to them including benefits and support services, advocating for their needs whilst empowering them to take ownership of their housing journey. You will work closely with the Landlord Liaison Officer who bridges the gap between our participants and private landlords. You will work closely with them to achieve prevention and relief outcomes by assisting the participant to remain in their provided accommodation.&nbsp;
You will provide follow up support to ensure tenancy sustainment and successful transitions into permanent housing, keeping casefiles up to date with activities and documents, and escalating any high risks or high needs to the management team.
About you
This role is ideal for someone who has a real passion towards helping to end the cycle of homelessness, providing support to those who need it to live in stable accommodation. You will thrive in a person centred environment with real compassion and a non-judgmental attitude, understanding that people face different life experiences and sometimes just need that extra support to get back on their feet. The Respite Care Team provides clients with opportunities to develop independence, build positive relationships and be empowered to make informed choices, within a person-centred approach.&nbsp; Respite Care offers brief respites to caregivers from their full-time caregiving responsibilities.&nbsp; No day may the same due to the rotation of service users requiring our service.&nbsp; Furthermore, the ideal candidate should possess the capacity to evaluate risks and devise strategies to mitigate and handle them effectively.
The working pattern will include working shift patterns including weekends and sleep-ins. A typical early shift may start at 7.00am during the week and 7.30am on Weekends and a late shift may start at 2.00pm with a 9.00pm finish or a 9.30pm finish if you are doing a sleep-in. You will be required to work two weekends in three.
You will be able to demonstrate:

<li>Good general education, to include good literacy skills</li>
<li>NVQ3/QCF Level 3 Diploma in Care and Promoting Independence or commitment to achieving this</li>
<li>A commitment to delivering Person Centred Support to a range of individuals</li>
<li>Able to relate to service users and meet their needs, with a sense of understanding, patience, respect and dignity</li>
<li>Ability to accurately maintain and review individual client files</li>
<li>Ability to undertake manual handling tasks</li>

The commissioning, contracting and market management, Residential and Nursing team work with a number of care homes across different user groups.&nbsp;
We are looking for a Market Oversight Manager to join us in the team.&nbsp;You are required to have good knowledge and experience of working with adults within care homes, collaborative commissioning and working with potential providers to consider innovative ways that services can be delivered.&nbsp;Understanding the Definition of Market Shaping, Commissioning&nbsp;Procurement and Contracting.
The role will require you to engage with stakeholders that reflect the range and diversity of communities and the range of needs experienced by people in its area.
You will work with partners to design, implement and manage projects that will improve the range of services available whilst ensuring that services developed, commissioned or procured are cost effective, efficient and designed to deliver good outcomes.
You will join a busy large, lively and dynamic team so the ability to work as part of a team and support other colleagues is required. The team has a varied function overall and the ability to think differently, creatively and have a “can do” attitude to work is desirable
The Person
You will be educated to degree level and have thorough knowledge of the principles of, and cycle of commissioning.
You are required to have an excellent understanding of project management methodologies and the ability to apply them effectively and a sound understanding of the policy and operating environment of health, social care, and regulated services.
As the role involves line management of staff so you will be required to have significant supervisory/line management experience and be a good communicator and&nbsp;information gatherer. You will also need excellent report writing skills and thorough understanding of the current legislative guidance.
We are looking for someone to join us who is a good communicator with a can-do attitude who wants to gain different experiences and knowledge in commissioning and working with care homes.
