Job Title: Advanced Practitioner - Preparing for Adulthood Team
Starting Salary: £48,226 plus £7,146 market enhancement, progressing to £53,460 plus £6,984 market enhancement pa – plus £1k one off welcome payment (subject to terms)
Hours: 37
Location: Apsley Hemel Hempstead or Farnham House, Stevenage (base location) with requirement to travel county wide - hybrid working model

About the team

The 0-25 Together Service works to ensure a seamless provision for disabled children, young people and young adults up to the age of 25.

We aim to make sure that our children, young people and their families are provided with the necessary information, advice and support to enable them to be as independent as possible.

Although 0-25 is within Children Services we also have accountability to Adult Care Services and this role will have to be able to work across both directorates.

This Advanced Practitioner opportunity is to work in the Preparing for Adulthood Team working with children as they move into adulthood. The Team work to ensure a seamless move linking with wider networks to support this.

About the role

You will manage and case work children and young adults with autism, learning and physical disabilities and will be able to lead on good practice and personalisation as our young people move into adulthood ensuring their transition is planned and co-ordinated. You will have a thorough knowledge and experience of EHCP’s and the social care role within this.

You will help to support and develop the staff team consisting of Preparing for Adulthood Practitioners.

You must have experience of EHCP’s and working with wider networks within these.

About you

Essential:

  • Registered social worker demonstrating a commitment to social work standards and sound value base.
  • Recent experience within a social work team and supporting the workload and allocations
  • Recent experience of supporting a duty system and being able to prioritise and respond flexibly to needs.
  • Experience of working with CYP with SEND who are transitioning into adulthood.
  • Knowledge of client finance, contributions, DRE and the implications of this as children reach adulthood.
  • Experience of involvement with EHCP’s and supporting with Tribunals with a thorough knowledge of the legislation.
  • Support and deputise for team manager.
  • Full UK driving licence with regular access to a vehicle.
