If so you may be the perfect candidate for this role!

The Knowsley Front Door Team is the first point of contact for all adults requiring advice or support from Adult Social Care. The team consists of varying staff from Call Handlers, ACM’s, Social Workers, Advanced Practitioners and Managers.

As an Assistant Care Manager in the Front Door Team, you will perform a multifaceted role from taking calls at the point of contact to screen or signpost to the appropriate services. You will participate in a duty rota, completing social care needs assessments for vulnerable adults and you will support social workers with the care management of a wide range vulnerable adults.

Ideally, you will have experience of assessing needs and taking enquiries from vulnerable people. You will be confident in using IT systems and have the ability to travel across the borough.

You will be expected to work flexibly, with a minimum of 3 office based days and 2 working from home, however it is priority to be able to meet the needs of the service however is required.

N.B. This role is subject to review and the duties may vary following this review.
