Supported Assistant Practitioner/Assistant Practitioner | 11257 | Permanent Contract | 37 hours per week | £30,023 to £32,046 per annum (F Grade) or £33,119 to £34,811 per annum (G Grade) | Kings Lynn

The Role.
You will have experience in the Health or Social Care sector and a good standard of education, along with excellent IT and organisational skills. You will have a positive, can-do attitude and be committed to building strong relationships with the people you support, their families, carers and partner organisations. You will use your professional curiosity to gather and share information, identify community resources, and support people to achieve their goals. Working within a strengths-based and relational practice framework, you will be able to work independently while also contributing effectively as part of a wider team.

With strong listening, communication and analytical skills, you will be flexible and adaptable to new ways of working. You will be committed to supporting adults to remain as independent as possible within their local communities, promoting wellbeing and positive outcomes through person-centred and relationship-based support.

This is a hybrid role. You will work across the local community, completing face-to-face visits, assessments and reviews in people's homes and community settings. You will also be required to attend your office base or another community setting for a minimum of two days per week to support collaborative working, team development and service delivery. The remaining time may be worked remotely where appropriate, so good broadband connectivity is essential.

Our Supported Assistant Practitioners (Scale F) join our 9-month, bespoke support programme (The Assistant Practitioner Development Programme), enabling you to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
