An exciting opportunity has arisen for a Bed Co-ordinator post based within Warrington's Adult Community Social Work, Locality Team but supporting community teams throughout Adult Social Care within Warrington.
We are working in partnership across the town to shape a health and social care system to be proud of, adopting a strengths based approach to the delivery of high quality care and support.
The Localities Teams provide an assessment & care management service working with adults in the community living with a range of long-term and complex health and social care needs including:-
- All adults living with dementia
- Older people living with significant hearing and/or visual impairments
- Older people living with frailty
- Older people who misuse alcohol or drugs
- Older people living with other debilitating health condition
The role
The Localities Service provides assessment, support planning and care management for adults living with a range of long-term and complex health and social care needs, including older adults living with frailty, dementia, sensory impairment and other significant health conditions.
This is a mixed role depending on the needs of the service which is ever changing and prioritisation is key. Although based within Locality team the bed coordinator will provide support across adult community services including working age adults, Intermediate Care and the First Response Team.
This role will include frequent communications with external providers to identify and coordinator timely admissions into 24 hour settings within the local area and surrounding areas. As well as clear and timely documentation and updating practitioners as required.
The Bed Coordinator will also be required to support on the duty team within the Locality Team as part of structured rota as well as when required by the service: Responsibilities will include:
- Responding to queries received via phone or email.
- Undertaking assessments of need and risk, including Mental Capacity Assessments and Best Interest decisions.
- Responding to referrals and urgent enquiries.
Therefore working between roles the successful candidate will need to evidence their ability to be flexible and also prioritise their work load.
What's needed?
Applicants must meet the essential criteria set out in the person specification. You must hold a full UK driving licence. The ability to communicate at ease with members of the public and colleagues, providing clear information and advice is essential for the post. DBS clearance is required.

The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
DBS clearance is required.
