
Care Manager | ASC Practitioner - Preparing for Adulthood Team (6-month Fixed-Term)
The pay range for this role will be £32,061 (pro rata for part time colleagues)

An exciting opportunity has arisen to join the Preparation for Adulthood Team as a Care Manager within Adult Social Care. This role is offered on a 6-month fixed-term, with a commitment of 37hours per week. As a Care Manager, you will collaborate closely with health and social care partners to enable recovery, promote independence, and support safe transitions of young adults.
Main Responsibilities
- Conduct assessments and reviews in line with the Care Act and Section 117, ensuring that all processes adhere to statutory guidance and best practice.
- Develop strengths-based plans and manage risk in partnership with multi-disciplinary colleagues, ensuring that planning is person-centred and collaborative.
- Maintain safeguarding practices and ensure that all records are kept to a high standard, reflecting accurate and up-to-date information.
The Ideal Candidate
- An enthusiastic individual with relevant experience in preparing for adulthood settings.
- Confidence in working within multi-agency partnerships and the ability to communicate in a clear and professional manner.
- Excellent communication, organisational, and digital record-keeping skills.
- A working knowledge of the Care Act, Mental Capacity Act, and strengths-based practice.
- To hold a valid driving licence and have access to a car
