As a Community Assessor, you will be responsible for providing practical and social support for vulnerable people and their families, including contributing to the assessment and review of care packages, recognising serious risk and promoting informed choices.

You will manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual support, whilst managing the reputation of the Council.

The successful candidates must be motivated and committed, with a range of skills and experience to strengthen established and developing teams.

Community Assessor role

The First Contact Community Assessor role involves undertaking assessments for individuals with care and support needs, as part of a proactive and innovative team. The postholder will receive a comprehensive induction alongside regular supervision and management support. The First Contact Team is responsible for all new assessment requests received by the Council, with further aspects of the role available for discussion.

What we are looking for

  • Level 3 qualification or equivalent in relating to working with Adults and Older People
  • Experience of working with people in need of support
  • Experience of completing assessments/risk assessments
  • Experience of working in social care
  • Experience of working with housing/health/ welfare benefits
