About the role

This is an exciting opportunity to join us in the Adult Social Care brokerage function as part of the Community Resource (Brokerage) Team - Fixed Term 1 Year contract.

What will you be doing?

You will provide operational services within the Community Resource team for all North Northamptonshire Council provider packages, to include community and acute for all adults over 18 years.

You will provide quality assurance to social care requests using a strength based model.

You will provide an effective, person focused support service to internal and external colleagues, to be responsive and a pro-active communicator.

You will be a motivated individual that is passionate about finding the right support for the individuals we broker care and support services for.
