We are looking for a Duty Community Care Officer to join our busy Assessment Teams at Gateway South and West, in the Connected Lives Gateway Service.
About the role
As a Duty Community Care Officer, you will support the Team Managers with tasks related to people referred into the service. This will be ensuring good communication with people & their families, their referrers and other partners involved with the person.
You will follow up on planned interventions from staff after assessments and the cease of their involvement, such as progress chasing the delivery of specialist equipment and ensuring the equipment is installed and meeting their desired outcomes.
You will hold strength-based conversations with adults or carers to understand their desired outcomes and explain how these can be met through the preventative and reablement pathway.
This post includes a high proportion of telephone contact with adults and carers referred to the team, seeking consent and establishing their views and reasons for referrals, to help the triage of the Team Managers in planning proportionate responses, ensuring the safety and well-being of the adult or their carers based on reasons for our involvement, nature, risk and complexity of their needs or those of their carers.
You will have knowledge of local resources, services and facilities and provide information, advice and signposting to prevent, delay or reduce care and support needs of adult or support needs of carers.
You will resolve individuals' needs through a range of creative interventions and connect them with wider social networks and community resources to meet some of their desired outcomes immediately or organize personalized solutions by collaborating with various partners.
Although you will be largely office based at Apsley, you may be asked to travel as part of the role to residents’ homes providing information, advice and support to prevent, reduce and delay their needs for care and support.
Specialist training will be provided to assess functional abilities and utilise assisted technology and equipment-based solutions.
This is a fast-paced team with clear solution focused short term involvements with a Reablement First focus.
About you
Essential Criteria:
- Full UK Driving Licence
- Previous experience in a Social Care setting
- Strong communication and negotiation skills
- Strong telephone skills
- A good standard of recording and ability to summarise findings
