As an Early Intervention & Prevention Practitioner - SEND, you will play a key role in delivering the Wakefield Families Together Best Start Family Hub offer, working with children, young people and families to provide timely support that prevents problems from escalating and improves outcomes. Working as part of a multi-disciplinary team, you will coordinate and facilitate the Team Around model, bringing together families and professionals to assess needs, develop support plans and review progress using a strengths-based, whole-family approach. You will work closely with schools, health services, social care, voluntary and community organisations to deliver evidence-based interventions, support families facing challenges, and ensure they can access the right support at the right time. The role includes coordinating Early Help referrals, chairing multi-agency meetings, supervising Hub Support Workers, maintaining high-quality records and contributing to service development and performance outcomes. We are looking for a skilled and motivated practitioner with experience of working with children, young people and families with SEND, excellent communication and partnership-working skills, and a strong understanding of safeguarding, Inclusion, SEND, child development and early help practice.
