Our Vacancy

We are looking for a compassionate and proactive individual to join the Integrated Discharge Team at Maidstone Hospital. In this role, you will help ensure patients can leave hospital safely and promptly by identifying and resolving barriers to discharge. Working closely with patients, families, healthcare professionals, and community services, you will support individuals transitioning from hospital back into their homes and local communities.

The role primarily supports patients at Maidstone Hospital, with occasional work at Tunbridge Wells Hospital, cottage hospitals, and step-down bed facilities. Home visits will also form part of your responsibilities to assess discharge needs and provide follow-up support.


What you'll do

You will work as part of the Integrated Discharge Team to support patients attending Accident & Emergency and those preparing to leave hospital following an inpatient stay. Working closely with patients, families, carers, healthcare professionals, and community services, you will identify and resolve barriers to discharge, provide advice and signposting, coordinate support services, and help ensure patients can return home safely and independently. The role includes undertaking home visits, assessing home environments, arranging practical interventions where required, maintaining accurate records, responding to enquiries, and promoting safeguarding, dignity, and confidentiality in all aspects of your work.


What you'll need

You will have experience working with vulnerable individuals in a health, housing, social care, or community setting, along with strong communication, problem-solving, and organisational skills. You will be confident assessing needs, coordinating support, and working both independently and as part of a multidisciplinary team. An understanding of safeguarding, confidentiality, and person-centred support is essential, as is the ability to manage a varied workload, maintain accurate records, and build positive relationships with service users and partner organisations. A flexible approach and willingness to undertake home visits and travel between locations will also be important.


This is a part time role working 14 hours a week, Monday and Tuesday 7 hrs per day (Either 9.00 - 5.00 or 8.30 to 4.30).

Please note this is a fixed term contract until 31st March 2027.
