About the role…
The aim of the team is to provide outreach support to customers with long term support needs, helping them continue living independently in the community and reducing the need for social care interventions. You’ll shape service delivery, build excellent partnerships, and champion continuous improvement so that our customers receive the right help, at the right time.
What you’ll do
- Lead, motivate, and develop a team of Outreach staff to deliver high-quality, person centred support.
- Build strong, productive relationships with external agencies and partners to maximise resources and opportunities for customers.
- Oversee case management—including timely assessments, risk management, and allocation of support.
- Drive continuous improvement by reviewing processes, policies, and working practices.
- Take a hands-on approach with complex cases and coach the team in good practice.
- Promote customer involvement and positively represent Peabody at forums and stakeholder meetings.
- Ensure accurate reporting, data quality, and effective use of systems to support service evaluation.
What you’ll need
- Previous experience managing and motivating a team.
- Strong organisational skills with the ability to prioritise a varied workload.
- Excellent communication skills (verbal and written)
- Knowledge of outreach support principles and the issues affecting vulnerable people.
- Confidence in handling challenging situations and applying risk assessment and de-escalation techniques.
- IT literacy and experience with record-keeping systems.
- The ability to build trust and collaborate with others.
- A full driving licence and access to your own vehicle is essential.
