Salary: Grade 5, £27,709 - £30,515 per annum. £14.36 - £15.81 per hour

37 hours per week

Your role:

As a Reablement Coordinator, you will play a key role in supporting the delivery of Wigan Council’s ‘Outstanding’ reablement service across the Wigan borough. You will coordinate and screen referrals by joint working with ‘Wigan Together pathway home’ team as Reablement representative. You will arrange assistive technology services and provide information and advice to professionals, people using services and their families. Working closely with health and social care colleagues, you will ensure timely interventions that support safe hospital discharge, promote independence and help people achieve the outcomes that matter most to them.

About you:

You are an organised individual, proactive and passionate about supporting people to maintain their independence. You have excellent communication and problem-solving skills, can manage competing priorities and are confident working both independently and as part of a multidisciplinary team. With good IT skills and attention to detail, you can maintain accurate records and respond effectively to referrals and enquiries. You are committed to delivering person-centred support, working collaboratively with colleagues and partners, and upholding high standards of safeguarding, confidentiality and customer service.

The Reablement Service is part of the Early Intervention Team, working alongside a range of health and social care professionals to support people to remain independent and achieve their goals. We focus on providing timely, strengths-based support that helps people recover skills, build confidence and continue living safely at home.
