Adult Social Care at Gateshead Council supports adults with care and support needs to live safe, independent and thriving lives. The service provides statutory assessments, safeguarding, prevention, and a wide range of community-based support in line with the Care Act 2014. Working in a strengths-based way, Adult Social Care focuses on wellbeing, independence and personalised outcomes, helping people to access the right support at the right time, whether through assessment and planning, enablement services, technology-enabled care, or wider community resources.

We're looking for a passionate and motivated Reablement Officer to join our dedicated team. This is an exciting opportunity to support adults with physical and mental health needs to achieve their goals, build confidence and maintain their independence. Combining hands-on case management, staff leadership and partnership working, you'll play a key role in delivering person-centred support that transforms lives. What you'll get to do:

  • Support adults with physical and mental health needs to maximise their independence and live fulfilling lives through intensive, short-term enablement support.
  • Carry out assessments and develop personalised support plans that focus on individual strengths, goals and outcomes.
  • Regularly review and adapt support plans to ensure they remain effective, appropriate and responsive to changing needs.
  • Manage a varied caseload, working closely with individuals, families and carers to deliver person-centred support.
  • Lead and support a team of Enablement Workers and Domiciliary Care staff, providing supervision, guidance and day-to-day operational management.
  • Coordinate rotas and workloads to ensure high-quality, efficient service delivery.
  • Work collaboratively with health professionals, social care teams, community organisations and other partners to provide a seamless service.
  • Identify and manage risks, ensuring the safety and wellbeing of service users and staff.
  • Support people and their families to confidently use equipment and assistive technology that promotes independence.
  • Maintain accurate records and ensure all assessments, reviews and service documentation are completed to a high standard.
  • Promote compliance with CQC standards, safeguarding requirements, and organisational policies and procedures.
  • Contribute to continuous service improvement and help ensure the delivery of high-quality, outcome-focused care and support.

What you'll bring:

  • NVQ Level 3 (or equivalent) in Care or willingness to work towards this qualification.
  • A willingness to undertake essential training in order to meet statutory requirements and develop competencies within the workplace (NB. This may take place outside usual working hours)
  • Understanding of relevant social care legislation, guidance and practice including CQC Compliance
  • Understanding the needs and welfare of vulnerable adults.
  • Experience of working in a care environment and/or community care services within a wide range of service users.
  • Ability to identify risks and complete assessments where appropriate and have the judgement to seek advice where required.
  • The ability to successfully deliver a customer focused service within aims and objectives of service provision.
  • Ability to work effectively and collaboratively as part of a wider multidisciplinary team.
  • Excellent written, verbal, organisational and IT skills, with the ability to communicate complex issues clearly and accurately and manage a diverse workload effectively.

If successful, this role requires a DBS and you will have to apply for an enhanced Disclosure.
