Overall Purpose of the Post:

SEMH Practitioner

  • Work as part of a multidisciplinary team to support children and young people with social, emotional and mental health (SEMH) needs, adopting a child-centred, strengths-based approach to improve wellbeing, resilience, engagement and outcomes.
  • Provide direct support and intervention to children, young people and their families, identifying needs, setting outcomes and delivering targeted SEMH strategies and support plans.
  • Undertake assessments and contribute to the planning, implementation and review of interventions, ensuring support is tailored to individual needs and promotes positive change.
  • Act as a key professional for allocated cases, coordinating support and working collaboratively with schools, families, health services, social care and other partner agencies.
  • Support education settings to identify and respond to SEMH needs through effective intervention planning, inclusive practice and the Graduated Response.
  • Gather relevant information and contribute to assessments of need to ensure children and young people receive timely and appropriate support.
  • Maintain accurate records, monitor progress and report on outcomes, ensuring compliance with safeguarding procedures, statutory guidance and local policies.

Keep up to date with developments in SEMH, SEND, safeguarding and education, applying current legislation, policy and best practice to support children and young people effectively

In accordance with the Council Probation Procedure, this post is subject to a 6-month Probationary Period.
