About the team
The Enablement Occupational Therapy Team provide support to older people and adults with a disability. We promote personalisation through our assessment process, focusing on the wellbeing of the individual.
Our Enablement Occupational Therapists deliver enabling programmes. Their input is time-limited, with intervention generally between 4 -6 weeks, this can be extended up to 12weeks where identified goals require this. The care management is completed externally to the enablement team allowing our Occupational Therapists to focus solely on their Occupational Therapy interventions.
About the role
- Working collaboratively with other partners and teams within Adult Care Services
- Developing innovative support plans
- Promote and support strength based assessment and practice approach to all interventions to maximise independence, health and well being to prevent or reduce the need for Adult Social Care
- Working creatively with residents to ensure their safety and independence
- Raising awareness in the area and developing valuable links for the team
- Supporting other Therapists and Community Care Officers
- Provide information and advice to enable an individual to confidently and safely participate in their daily life, includes grading of tasks and activities and adapting their environments
- You will undertake a specialism eg Moving and Handling, BiA, AMP or Occupational Therapy practice educator
- Designing personalised programmes that enable, and encourage the individual to relearn skills and regain abilities.
- Reviews of moving and handling, provision of specialist equipment, & minor adaptations to support identified goals.
- Supporting people in the community and on discharge pathways from acute hospital settings
About you
Essential:
- HCPC Registered Occupational Therapist
- Full driving licence and regular access to a vehicle
- 2 years post qualifying experience as an Occupational Therapist
- Knowledge and experience in social care sector
- Experience of completing assessments and making recommendations
- Ability to work on your own as well as part of a team
Desirable
- Knowledge of Care Act 2014
- Knowledge of Mental Capacity Act 2005
