About the Role

We are delighted to offer an exciting opportunity for a motivated and enthusiastic Occupational Therapist to join our Integrated Care Funding Team within the Adults Social Care Department at Backburn with Darwen.

This role involves delivering essential community-based assessments and interventions for individuals in receipt of formal care and who are requiring equipment and minor adaptations to support their independence and well-being.

We are seeking someone who is passionate about delivering high-quality outcomes and values working within a small unitary authority where trust, respect, ambition, collaboration, and kindness are central to everything we do.

Key Responsibilities

The successful candidate will undertake a varied and rewarding role, including:

• Conduct community Occupational Therapy assessments in a local authority setting, identifying needs, recommending standard equipment and minor adaptations, signposting to relevant services and offering support and guidance to promote independence, health and wellbeing.

• Collaboratively work with residents, families, Social Workers, Assessment and Review Officers and strategic partners to create individualised support plans that are proportionate and least restrictive.

• Maintain thorough and accurate records in line with professional and legal standards
• Contribute to service development and improvement

About You

We welcome applications from:

• Experienced Occupational Therapists with a minimum of two years’ experience in community, or health settings, particularly working with physical disabilities across a range of specialities.
• HCPC registration and relevant qualifications in Occupational Therapy.

You will demonstrate:

• Excellent communication and interpersonal skills
• Strong clinical assessment and decision-making abilities
• The ability to work independently while also working collaboratively with a team of social workers and Assessment and Review Officers.
• Experience in working within a local authority setting.
• Proficiency in completing manual handling assessments.
• Strong knowledge of equipment provision and home adaptation processes.
