About the team
This is an exciting opportunity to be part of the Brokerage Solutions Team within Hertfordshire County Council. BST are recruiting for a permanent Flexible Service Finder.
This is a county wide post based in Stevenage or Apsley and also be able to work remotely from home.
About the role
As a Flexible Service Finder, you will be working within Brokerage Solutions Team which provides the brokerage function across Hertfordshire for the Adult Disability and Older Peoples Services. The team is part of the Community Commissioning Team for Disabled Adults (CCDA).
The Flexible Service Finder role sources and commissions services to meet the individual needs of Elderly or Disabled Adults with a learning or Physical disability/Autism diagnosis /Mental Health diagnosis or combined co morbidity. Individuals may be referred from both Adult Disability Service (ADS), the 0-25 Together team (0-25), the Older People’s (OP) teams or Hospital teams.
The role is full time and will require a 9 - 5.30 working pattern (4.30 PM Fridays) due to the business nature of our provider partners
You will need to demonstrate where you have developed good working relationships with our care providers and have a thorough knowledge of the services they offer.
You will be working closely with our ADS, OP, 0 – 25 Together Service, & hospital teams and in some cases will have direct contact with Older people’s families.
You will be required to work in partnership with Commissioners and finance teams to ensure appropriate and effective provision is sourced and commissioned and finance forms are completed accurately.
About you
Essential:
- Experience of Brokerage Service
- Experience in Social Care delivery
- Good I.T Skills, Microsoft Excell, Word and Power point proficient
- Organisational skills, ability to work remotely and manage own workload
- Evidence of numeracy skills, calculating and negotiating costings with providers
Desirable:
- Driving license and access to a vehicle
- Social Care contract knowledge
