Short Breaks Practitioner
We are seeking a Short Breaks Practitioner to join our Short Break Assessment Team in Wakefield.
The team supports children and young people with Special Educational Needs and Disabilities (SEND) to access meaningful activities, develop independence, build friendships, and engage with their local communities.
Working closely with children, young people, families, and partner agencies, the successful candidate will undertake assessments of individual needs and coordinate creative, flexible, and practical packages of support. The role is focused on achieving positive outcomes for children and young people with SEND, enabling them to reach their full potential while supporting family wellbeing and resilience.

Salary range
- £33,119 - £35,570 per year
