    We are seeking a dedicated and compassionate Social Care Coordinator to join our dynamic Physical Disability and Older People Team within Adult Social Care. This is an exciting opportunity to make a meaningful difference in the lives of adults in need of care and support.

    About the Role:
    As a Social Care Coordinator, you’ll play a key role in supporting residents to access the right care and services at the right time. You’ll undertake assessments, contribute to support planning, and work collaboratively with colleagues, families, and partner organisations to achieve positive outcomes for the people we support.

    Your role will involve:

    • Completing Care Act assessments and reviews under the guidance of qualified social workers.
    • Developing and implementing person-centred support plans.
    • Coordinating care packages and liaising with providers and external agencies.
    • Promoting independence and wellbeing through a strengths-based approach.
    • Maintaining accurate and timely case records in line with statutory requirements.

    About the Team:
    The Physical Disability and Older People (PDOP) Team is part of the Living Well Service within Adult Social Care. We work closely with other teams across the Council to deliver high-quality, person-centred support to adults aged 18 and over with long-term physical disabilities, and to residents aged 65 and above who have care and support needs.

    Our multi-disciplinary team includes Occupational Therapists, Social Workers and Social Care Coordinators, all working together to assess needs, plan care, and empower individuals to live as independently as possible.

    Our Vision:
    To maximise the quality of life of Reading residents who have a long-term physical disability or are over 65 years of age and have Care Act eligible needs, using a strengths-based and problem-solving approach.

    About You:
    We’re looking for someone who is compassionate, organised, and committed to supporting vulnerable adults to live fulfilling lives. You’ll have experience of working within a health or social care setting and a good understanding of the Care Act and adult safeguarding principles.

    You’ll also need to be:

    • A strong communicator with excellent interpersonal skills.
    • Able to work effectively as part of a multidisciplinary team.
    • Confident using IT systems for recording and managing information.
    • Passionate about delivering high-quality, person-centred care.
