
Social Care Practitioner (2 Posts)
We are a long-term generic adult team, working with people from 18 (or approaching 18 for transition cases) and their carers, covering the South of Amber Valley. The people we work with have social care needs resulting from a range of disabilities people with learning disability and autism, physical disability, and dementia. The successful candidate will have a working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Using person centred, strengths-based approaches we support people to identify and achieve their goals with a home first approach. Successful candidates must have knowledge and experience of making safeguarding personal.
The team is busy yet supportive. The workers joining us will need the ability to prioritise competing demands, manage their time effectively, be resilient and reliable. The role includes assessing and reviewing eligible care needs and working with people and their support networks to create support plans, commissioning services, covering duty, conducting safeguarding enquires and the ability to travel.
The successful candidates will be given opportunity for daily support and peer group discussion and will be provided with regular professional supervision where you will be supported to achieve best practice. We operate a ‘champion’ role offering opportunity to develop and share your knowledge and practice interests. You are required to work at least one day a week in the office. Working at home arrangements and flexi working are part of this role.
This is a temporary post for 12 months with the possibility of extension but no guarantee.

Salary range
