Social Care Practitioner - Mental Health

About the TeamThis is a unique opportunity for a Social Care Practitioner to be part of an integrated multi-disciplinary specialist mental health team. The successful applicant will be employed by Warwickshire County Council but seconded to Coventry and Warwickshire Partnership Trust (CWPT) Mental Health Services under a successful section 75 agreement. This allows for the meaningful integration of the social care agenda within the local Mental Health Trust and prioritises a holistic person-centred approach to meeting the needs and outcomes of the local population.The population of South Warwickshire is over 250,000 and includes the historic market towns of Warwick and Kenilworth, the regency splendour of Royal Leamington Spa, Shakespeare’s birthplace Stratford-upon-Avon as well as hundreds of acres of beautiful Warwickshire countryside.About the RoleYou will play a crucial and valued role in the South Warwickshire Community Mental health & Well-being Team. You will take a strengths-based approach to supporting people with a range of mental disorders including schizophrenia, depression, anxiety, OCD, autism and personality disorder; maximising their potential for independent living and promoting their full inclusion in the community. You will be able to demonstrate excellent organisational skills, a head for complexity, and a desire to improve the outcomes and wellbeing of those you work with.Key Requirements· You’ll have at least 2 years’ experience of working with people with complex and significant mental health problems including autism. This role serves as an ideal opportunity to further develop specialist skills.· You will need strong analytical and communication skills and be able to work on your own initiative as well as collaboratively with colleagues. With the ability to work to tight deadlines you can manage under pressure as well as maintaining composure and your professional stance; you’ll be used to making difficult decisions.· You’ll be able to influence and persuade others as well as being flexible and innovative in your approach.
