Following a service wide review of adult social services, and development of staff in the service, we have vacancies across the service.

The Social Care Connect Team are looking to recruit:

• 1 x Full time permanent social workers working over a 5 day working pattern (Monday- Friday)
• 1 x Part time permanent social worker working over a 7 day working pattern (23.5 hours per week)

We work with individuals aged eighteen plus, this means that our work is varied and changeable, and covers individuals with a variety of different needs. We cover hospital discharge work under the 'discharge to assess' model and we take referrals from the community.

The Older People and Physical Disabilities Team are looking to recruit:

• 3 x Full time permanent social workers working over a 5 day working pattern (Monday to Friday).

We support individuals aged 18 plus.

The successful candates will:

• hold a social work degree and be registered with Social Work England.
• be a strong team player, resilient and solution focused with a proactive approach to work as part of a busy Social Work Team. (These positions are open to experienced workers and Newly Qualified Social Workers and we offer an ASYE year)
• be highly motivated and willing to undertake Care Act Assessments, support/care planning and commission services with person centred outcomes.
• be able to provide social work supervision and have experience of social work management.
• be able to travel throughout North Lincolnshire independently, as the team covers all of the North Lincolnshire area.
• be knowledgeable of adult social care legislation and frameworks
• need to demonstrate an ability to problem solve and have good negotiation skills, along with effective communication, organisational and IT skills.
• need to be flexible, resilient and dedicated to team work.
