Our Team:

Complex Care Widnes works with adults aged 18 and over. The needs of people with lived experience are diverse and may cover learning disabilities, physical and sensory disabilities (including neurological conditions and acquired brain Injuries) and older people. The team is made of a Principal Manager, Practice Manager, Advanced Practitioners, Social Workers and Community Care Workers

The focus across the teams’ processes is to apply strengths-based approaches in the course of assessment/reassessment of needs. An emphasis is placed on signposting and use of informal networks ahead of commissioning care packages.

You must be a qualified Social Worker who is registered to practice with Social Work England, demonstrating a commitment to ongoing professional development. In Halton, the post will provide you with the opportunity to develop and enhance your skills in Social Work. And be part of the development of partnership working with health colleagues to enhance the experience of adults who use adult social care services.

You must have knowledge of Section 42 Safeguarding Enquiries, Court work, completing Risk Assessments as well as experience of working closely with a range of services, stakeholders and professionals. Our post a carries an essential car user role, and you will need to hold a current full driving license and have access to a vehicle.

Hours: 35 per week
