
Support Worker (Hospital Discharge Service -Mental Health)

We are recruiting a full‑time Support Worker to join our team. We provide specialist housing, care and support to people who are socially excluded, disabled, unwell or homeless. In this role, you’ll deliver high‑quality and effective hospital discharge services that promote independence, wellbeing and inclusion, while working in line with Peabody’s values, policies, procedures and best‑practice standards.
You will need to demonstrate good knowledge of support tools and an understanding of the needs of people being discharged from hospital with complex needs. Excellent communication and IT skills are essential.
This role will require an enhanced DBS check.
