About the Role: Transition Support Practitioner

  • We are seeking a dedicated and compassionate Transition Support Practitioner to join our team. This is a rewarding opportunity to support children and young people who face barriers to learning, attendance, and successful engagement in school.

    The Transition Support Practitioner will provide a complementary service to schools and advisory teachers, addressing the needs of children who require assistance in overcoming barriers to learning and accessing school in order to achieve their true potential. The role will support reducing incidences of both internal and external exclusion.

    Working across a range of educational settings, the successful candidate will support pupils with diverse needs, whilst prioritising those who require the highest levels of intervention and support and working alongside families.

Key Responsibilities:

  • Work directly with children and young people who are experiencing barriers to learning, attendance, engagement, or successful inclusion within school.
  • Deliver targeted interventions to support emotional wellbeing, resilience, social communication, self-regulation, and positive behaviour.
  • Facilitate successful transitions into, between, and within educational settings.
  • Build positive, trusting relationships with pupils, families, school staff, and external agencies.
  • Support the development of strategies that enable pupils to access learning, participate in school life, and achieve positive outcomes.
  • Assist in reducing the risk of internal and external exclusion through proactive and preventative support.
  • Work collaboratively with advisory teachers, SENDCOs, pastoral teams, school leaders, and other professionals.
  • Contribute to assessments, reports, support plans, and reviews as required.
  • Maintain accurate records of interventions, outcomes, and pupil progress.
  • Promote safeguarding, wellbeing, equality, diversity, and inclusion in all aspects of practice.
  • Support pupils both individually and in small groups within school and community environments.
  • Travel between schools and settings across the local authority area as required.

What will be expected from me?

  • Has experience of working with children and young people in educational, pastoral, youth work, social care, or related settings.
  • Has a sound understanding of Special Educational Needs and Disabilities (SEND), particularly social, emotional and mental health (SEMH) needs.
  • Can build positive and effective relationships with children, families, schools, and other professionals.
  • Possesses excellent communication, interpersonal, and listening skills.
  • Demonstrates patience, empathy, resilience, and a child-centred approach.
  • Has experience of supporting behaviour for learning and promoting positive outcomes for pupils.
  • Can work independently whilst also contributing effectively as part of a wider team.
  • Is able to plan, deliver, monitor, and evaluate targeted interventions.
  • Has good organisational skills and the ability to maintain accurate records.
  • Understands safeguarding responsibilities and is committed to protecting children and young people.
  • Demonstrates a commitment to inclusion and helping every child achieve their full potential.
  • Is confident using IT systems for recording information and communicating with stakeholders.
  • Holds a full UK driving licence and has access to a vehicle for business use.
