We have opportunities within our adult disability, older people and hospital teams across Hertfordshire.
Prevention of Admissions Team - Hemel Hempstead
Join our well-established, integrated Health and Social Care Prevention of Admissions Team, based in Hemel Hempstead, and be part of a dynamic, multidisciplinary service dedicated to keeping people safe and well at home.
You’ll work alongside a skilled team of nurses, paramedics, physiotherapists, and occupational therapists, delivering rapid, person-centred care to residents across South West Hertfordshire.
You'll be part of:
- A collaborative, co-located team environment with health colleagues
- A fast-paced service providing urgent 2-hour response assessments in community settings
- A focus on preventing unnecessary hospital admissions, helping individuals remain in the comfort of their own homes
- Operating hours: Monday to Friday, 9:00 AM – 5:30 PM (4:30 PM on Fridays)
Gateway Team West
Be part of the front door to adult social care in Hertfordshire, focusing on prevention, early intervention, and short-term support.
This role involves traveling throughout Hertfordshire to visit residents' homes and the office, providing crucial information and support to prevent, reduce, and delay the need for statutory services. You will work within a 12-week pathway to connect people with resources that empower them to manage their lives independently.
You'll
- Conduct assessments and reviews (including carers)
- Develop personalised solutions, and coordinate support to maximise independence.
- Collaborate with various partners while adhering to professional standards and legislation
Specialist training will be provided to assess functional ability and utilise technology and equipment-based solutions. You will .
Adult Disability Team
This service supports adults with learning and physical disabilities to live independently and engage with their communities.
You’ll:
- Complete care needs assessments
- Develop innovative, person-centred support plans
- Build networks and raise awareness locally
- Work alongside community care officers, learning disability nurses, and occupational therapists
This is a fast-paced, community-focused environment. A full UK driving licence is required. Hybrid working is available, with a minimum of 2 days per week in the office.
Hospital Integrated Discharge Team
- market forces enhancement per year
- 5% operational enhancement.
These enhancements are non-contractual payments, subject to a 3 yearly review
You'll ensure safe and timely discharges from hospital by:
- Assessing the needs of patients and carers
- Arranging appropriate care packages
- Liaising with professionals and agencies
- Supporting person-centred discharge planning

Herts Valleys Community Hospital Team - 24 hours per week
Older People Team
Support adults and carers with long-term health and social care needs. Our teams work under the Care Act to safeguard, assess, plan, support, and review care needs. Roles are available in:

- Residential and community reviewing teams
- Crisis intervention and hospital admission prevention teams
- Sensory services
